Children 5yrs and under will be bulk billed. Pensioners and Health Care Card holders will be bulk billed upon presentation of a current Pension or Health Care Card. Workers’ Compensation consultations will be billed to the relevant insurer upon receipt of a claim number. Pre-Employment Medicals (PEMs) will be billed to the requesting body as pre-arranged. All other patients will be charged a private fee. If you do not hold a current Medicare card you cannot be bulk billed – standard rates apply.

Reception staff will submit a claim to Medicare on behalf of eligible patients at the time of payment. It is recommended that patients contact Medicare to ensure that their bank and postal details are up to date.

Reception staff will advise patients of out of pocket expenses for consultations, procedures and additional tests whenever possible.

If you are unable to pay your account due to financial hardship please discuss this with either your doctor or the Practice Manager so that alternative arrangements can be made.

Payment is expected at the time of consultation. We accept cash, EFTPOS, Master Card and Visa.

APPOINTMENTS

Bookings are essential for non-urgent matters, however we understand that illness does not run to a schedule; therefore we hold appointments daily for these events.

If you require urgent care please inform the Receptionist, who will triage and advise accordingly.

EMERGENCY CARE

In the event of an emergency please ring St John Ambulance on 000 or present to Gove District Hospital for immediate care.

HOME VISITS

Home visits are available at the doctor’s discretion. A private fee will be charged for home visits.

AFTER HOURS

For all urgent after hours care present to Gove District Hospital.

INTERPRETER SERVICES

All Doctors at this practice are registered with TIS National. Should you require interpreter services please advise Reception staff at the time of booking.

PRESCRIPTIONS

For medico legal reasons doctors encourage patients to attend for consultation to obtain repeat prescriptions; however, in some instances a repeat prescription will be issued outside consultation. This script will incur a private fee as detailed in the fee list.

Most results are received within five days, whilst others can take up to ten days, depending on the test requested. It is important to make a follow up appointment with your doctor on the day of collection, as this will help with appointment availability. The doctor will contact you prior to your appointment should your result be of a more urgent nature.

TELEPHONE CALLS

Patients are required to make an appointment to discuss health issues with their doctor; however in some circumstances it is reasonable to request a call back. Details of the call back are to be left with the Receptionist. The doctor will return your call at their earliest convenience.

PATHOLOGY

Endeavour Health Services collects pathology on behalf of Western’s Diagnostic Pathology. Appointments are available between 8.30am and 10.30am, Monday through Friday. It is essential that you present to this appointment with your pathology request form.

Whilst most pathology tests are covered by Medicare there are some that are not. Western’s will invoice you directly for these.

WAITING TIMES

We appreciate that your time is valuable and try to keep waiting times to a minimum; however consultations can often go beyond the allocated time. Please assist by booking a long consultation if you have several matters to discuss with your doctor or you are having a procedure. The practice has set the fees to ensure that out of pocket expenses for long consultations are similar to that of a standard consultation.

Additionally, each patient is allocated an appointment time. Please refrain from fitting additional family members into a single appointment, as this may also cause the doctor to run late.

REFERRALS

Please provide Reception staff with referrals addressed to visiting Specialists whom attend the Practice. Reception staff will forward referrals to the relevant Specialist prior to the next scheduled visit date.

Reception staff will fax and forward referrals (by internal mail) to Specialists attending Gove District Hospital if requested.

Patients are responsible for ensuring all other referrals are presented to the Specialists at time of consultation.

COLLECTING & MAINTAINING INFORMATION

In the provision of clinical services the Practice will need to collect your personal information. Personal information collected includes name, address, contact details, Medicare number, healthcare identifiers and clinical information such as medical history, medications, allergies, adverse events, immunisations, social history, family history and risk factors. The procedure for collecting your personal information commences upon registration and will continue during the course of providing medical services. Your collected personal information can be held in either electronic or paper form. Practice staff will endeavour to keep patient information up to date and you may be asked to verify information when you attend. Alternatively, you may request a correction or update to your file. In this instance a written request is required. The practice evaluates all unsolicited information it receives to decide if it should be kept, acted on or destroyed. The practice holds all personal information securely.

DISCLOSURE OF INFORMATION

The Practice will NOT disclose personal information to any third party other than in the course of providing medical services, without full disclosure to the patient or the recipient, the reason for the information transfer and full consent from the patient, unless, required by Law, to prevent a serious threat to a patient, assist in locating a missing person, to defend an equitable claim or as part of a confidential dispute resolution process. The Practice will not disclose personal information to anyone outside Australia without need and without patient consent.

PATIENT ACCESS TO MEDICAL RECORDS

Patients are entitled to view their medical records but an appointment will be required with a doctor and a fee will apply. Please obtain a ‘Request to Access Medical Record’ form from Reception if you wish to proceed.

TELEHEALTH CONSULTATIONS

Telehealth consultations are available at this practice. This enables patients to have consultations via a video link with eligible specialists, consultant physicians or psychiatrists. If you feel that this would be beneficial to your health care and would like more information regarding this service please discuss this with your doctor.

FILE TRANSFER

If you require your medical records to be transferred to another practice, we will require a completed request form from that practice signed by yourself. A fee will be charged for this.

RECALLS

Endeavour Health Services has a number of recall systems in place. These are actioned regularly by the Practice Nurse. If you are required to attend for care or follow up the Practice Nurse will contact you to arrange an appointment. The practice also participates in State, National and Territory reminder systems. If you do not wish to be part of this process, please notify your doctor or Receptionist.

ELECTRONIC COMMUNICATION

Email is not used by this practice for the transmission of clinical information.

eHEALTH

Endeavour Health Services participates in eHealth. Only registered patients can access this service.  Patients can registrations online by visiting ehealth.gov.au.
